Failed automation is useful when analysed. Common causes are inconsistent source processes, unclear inputs, missing owners, no escalation route, weak monitoring, and treating model output as final truth.

Map the existing workflow before automating it. Identify decisions, exceptions, source systems, accountable people, and a fallback process. Automate one stable step, monitor errors, and expand only after it works reliably.
