Human approval is useful when an action is difficult to reverse, affects money, grants access, changes customer data, makes an external commitment, or relies on uncertain information.

A simple design is to score each action by impact and confidence. Low impact plus high confidence may proceed automatically. Everything else creates a clear approval request containing the proposed action, source evidence, and reason.
