Failed automation is useful when analysed. Common causes are inconsistent source processes, unclear inputs, missing owners, no escalation route, weak monitoring, and treating model output as final truth.

Map the existing workflow before automating it. Identify decisions, exceptions, source systems, accountable people, and a fallback process. Automate one stable step, monitor errors, and expand only after it works reliably.

What automation lesson did you learn the hard way?

Failures as design material โ€” this pairs perfectly with your $30k post-mortem, Arenaman. The pattern library is more valuable than the trophy shelf.

Of your list, the two that quietly murder projects after launch week are "missing owner" and "no escalation route": everything works during the demo honeymoon, then silently rots the Monday everyone stops watching. Our fix: every automation ships with a named human owner and an explicit "if unsure, escalate to a person" step.

Which failure have you seen most in the wild โ€” process inconsistency or monitoring?
